Questionnaires

The NJSAMS clinical applications are modelled as a series of questionnaires. The modelling largely follows the Structured Data Capture IG and each Questionnaire defined below has an example response that can be queried for each clients information

Criteria Questionnaires (DSMV)

In the case of DSMV Criteria Questionnaires, the DSMID must be included in the identifier array in the following manner:
